Requirements:- At least a High School Graduate
- 18 years old and above
- PSA (photocopy only)
- 5 pcs. Passport-size photos (white background, with collar and nametag)
- 5 pcs. 1×1 ID picture
- 5 photocopies of 1 valid ID (with 3 specimen signatures)
- Form 137 or Transcript of Records (TOR)
- Marriage Certificate (for married women only)
- Long brown envelope and a black ballpen
